ECRI suggests the following emergency procedure
when extinguishing airway fires. This procedure should be reviewed
prior to each surgical procedure.
At the first sign of an endotracheal tube fire, disconnect the
breathing circuit from the tube. Signs of such a fire are rocket-like
flames shooting from the tube or an orange or red glow from the
tube or a darkening of the breathing circuit,
Second, remove the endotracheal tube and have another team member
extinguish it. Next, remove any cuff protective devices and any
segments of burned tube that may remain smoldering in the airway.
Reestablish the airway after clearing it of any residual debris
and resume ventilation with air. Do not administer 100 percent
oxygen until it is absolutely certain that nothing burning remains
in the airway.
Finally, examine the airway for the extent of damage and treat
the patient accordingly.
If a fire should occur, all materials, devices and equipment involved
in the incident should be saved for later investigation.
